Application Process

The College of Nursing admits students to the DNP–Nurse Anesthesia Emphasis every summer. The application is available through Dec. 1 for the following summer admission cycle. Application materials must be received by the College of Nursing Graduate Program Office, and official transcripts must be received by the UW Oshkosh Graduate Studies Office no later than Dec. 1 to to be eligible for the following summer start date.

Step 1 – Apply to the University

Complete the online UW System application and pay the $56 application fee at apply.wisconsin.edu.

Once completed, the application is sent electronically to UW Oshkosh Office of Graduate Studies. The Graduate Studies office will review and forward the online application to the College of Nursing.

Application must be received by Dec. 1 to be eligible for classes the following summer.

Step 2 – Submit Official Transcripts

Submit official undergraduate transcripts (and graduate, if applicable) to UW Oshkosh Graduate Admissions (gradschool@uwosh.edu) by Dec. 1. Transcripts must be sent electronically, directly from a university; transcripts submitted by the applicant are not considered official.

Step 3 – Submit Supplemental Materials

Applicants must send items A–D and F directly to the College of Nursing Graduate Program Office via email to conna@uwosh.edu. Send all items, combined as one pdf, as an attachment in a single email by December 1st. 

Items E, G and H are to be sent via email to conna@uwosh.edu by your supervisor, reference or CRNA followed in job shadow.

  • All items are due by December 1st.

A) Resume listing professional association memberships, certifications and continuing education activities – please attach copies of any certifications you have including BLS, ACLS and CCRN

B) RN license (copy from credentialing website is accepted)

C) CCRN (copy of CCRN is required)

D) Background Information Disclosure form (BID)*

Note: Applications can be submitted by those who have yet to graduate from a BSN program. Transcripts bearing BSN degree completion must be submitted prior to receiving full-standing admission to the program.

*Upon admission to program, applicants must complete a full background check with associated fees. Background checks are not considered part of the evaluation/selection process and are only used for advising purposes.

E) Applicant Clinical Experience Form – To be completed by current supervisor and emailed to conna@uwosh.edu

F) Applicant Critical Care Experience Form – To be completed by applicant and included in ONE email with items A-D.

Reference Forms

Applicants are to complete the top portion of the reference form (Item H) and forward to the three references they have listed on the UW System online application. Reference forms are to be sent directly from the references to the College of Nursing Graduate Program Office by email to conna@uwosh.edu by December 1st.

G) Reference Letter Form – A minimum of three references forms are required and can be emailed to conna@uwosh.edu:

  1. Current supervisor
  2. Academic instructor**
  3. CRNA followed in job shadowing.

**Applicants who have been away from school for an extended period of time should utilize a reference who can summarize the applicant’s current knowledge base.

H)  CRNA Shadowing Verification Form.  This form can be emailed to conna@uwosh.edu along with their reference.

Step 4 – Admission Interview

After admission requirements are met and supporting materials have been received by the respective graduate offices by the Dec. 1 deadline, eligible applicants will be invited to participate in an interview.
