BSN to DNP:

Nurse Anesthesia Emphasis – Application Process


The College of Nursing admits students to the Nurse Anesthesia Emphasis every summer. The application is available until December 31st for the summer admission cycle. Application materials must be received by the College of Nursing Graduate Program Office and official transcripts must be received by the Graduate Studies Office by December 31st to to be eligible.

Complete the online application and pay the $56 application fee at http://apply.wisconsin.edu.  The application is a UW System form and is sent electronically to UW Oshkosh Office of Graduate Studies. This MUST be completed by December 31st. The Office of Graduate Studies office will review and forward the online application to the Graduate Program Office in the College of Nursing.

Submit official undergraduate and any graduate transcripts to Office of Graduate Studies by December 31st for processing. Transcripts must come directly from the university to the Office of Graduate Studies; transcripts submitted by the applicant are not considered official:

Applicants must mail Items A.-E. directly to the College of Nursing Graduate Program Office.

A.  Resume (please include your professional association memberships, certifications, and continuing education activities).

B.   RN license  (copy from credentialing website is accepted).

C.  A 250-word essay describing what you have done to prepare yourself for graduate study, why you are applying to this particular program, and what are your career goals post-DNP.

D. Background Information Disclosure Form (BID)* – Print and complete this form.

E. BSN DNP Written Admission Requirement.

Applications can be submitted from those who have yet to graduate from their BSN completion program.  Transcripts bearing BSN degree completion must be submitted prior to granting full-standing admission to the program.

*Upon admission to program, applicants will be requested to complete a full background check with associated fees the student’s responsibility. This form will not be part of the evaluation process for selection into the program, but will be used for advising purposes only.

F.  Applicant Clinical Experience Form – Form to be completed by current supervisor.

G.  Applicant Critical Care Experience Form Form to be completed by applicant.

Reference Forms

Applicants must follow directions for reference form (Item F) for completion of application process. Instructions: Applicants are to complete the top portion of the reference form and forward to the three references they have listed on the Office of Graduate Studies online application form. Reference forms are to be returned directly from the references to the College of Nursing Graduate Program Office.  This can be done either by email, fax, or mailing to the address listed at the bottom of the form.

H. DNP-CRNA_Reference-Letter-Form – A minimum of 3 references forms are required: 1. Current Supervisor, 2. Academic Instructor, ** 3. CRNA followed in job shadowing.

**Applicants who have been away from school for an extended period of time should submit a reference that summarizes current knowledge base.

After admission requirements are met and submitted by deadline dates, eligible applicants will be invited to participate in an interview.
