Case Management Services

The mission of our Case Management program is to support students by providing resources, referrals, and advocacy on various needs. We strive to provide quality referral information in a timely manner taking into account the individual needs of each student.

 

What We Help With

  1. Basic needs (food pantries, medical insurance, shelter, etc.)
  2. Counseling needs (if it is determined that a student needs specialized mental health support)
  3. Medical needs (provider referrals, addressing financial barriers for care, etc.)
  4. Community Resources (support groups, interpersonal violence support, etc.)

How to Make an Appointment

Call the Counseling Center at (920) 424-2061 and request a case management appointment with the Case Manager.

Please be sure to bring your insurance card to the appointment to assist with a timely referral.

ADHD

The counseling Center no longer provides ADHD evaluations, so if you are in need of this, please request a case management appointment with the Case Manager.
