Discipline File Document Request Form

Because your Conduct File is a part of your educational record, you are entitled to view your file, and if requested, obtain copies of items within your file. In order to request information from your Conduct File, please note the following information:

• All requests must be made at least five business days in advance.
• If you request a copy of your entire file, you will be charged an administrative fee of $10.00, which must be paid before you receive your copies.
• Any information pertaining to other students will be removed from the records that you receive.
