Undergraduate Students

Step 1: Complete the online form to request to withdraw or cancel enrollment.

Step 2: Participate in an individual meeting with an Enrollment Management team member.  You will be contacted within 2 business days after this form is submitted to setup this meeting.

*Withdraw requests will only be approved after the completion of your individual meeting.

 

Graduate Students

Step 1: Complete the online form to request to withdraw or cancel enrollment.

Step 2: Your form will be forwarded to Graduate Studies for approval.

This form should be completed if you are requesting to drop/withdraw from all second 7-week courses, interim or if you are planning to complete this semester and not return for the Fall 2019 term.

If you are wishing to drop/withdraw from fourteen week or first 7-week courses, please complete a request for a late drop/withdrawal through the Dean of Students office.  The form can be located here: https://www.uwosh.edu/deanofstudents/documents/late-drop-late-withdrawal-form
