Group Pack Out Request
- This form must be completed and received no later than three working days before the pick up date
- Requests submitted later than the three day notice will not be accepted
- Names of people participating must be listed on the back side of this form.
- Those using Titan Dollars will have the cost of the meal deducted from their balance
- All food will be picked up at the Blackhawk Commons kitchen. See a manager
- The group coordinator must make all changes including cancellations with a Blackhawk Commons food manager at 424-3482 no later than 48 hours before the scheduled pick up date, Monday through Friday between 9am-5pm
