UWO Change of Campus Form

The following form is for current students at the campuses of UW Oshkosh ONLY!

Are you looking to switch from your home campus, to another (Oshkosh, Fox Cities, or Fond du Lac)? You’ve come to the right place.

Students must be an enrolled, degree-seeking student at one of the UWO campuses (Oshkosh, Fond du Lac, Fox Cities) to complete this form. Students looking to transfer from a non-UW Oshkosh campus should follow these steps.

Students on all three campuses of UW Oshkosh should complete the following form to start the process of changing campuses. It takes approximately two weeks to process requests.  Once a student’s home campus has been officially changed, the student will receive a confirmation email with additional information.  If you have any questions regarding the form or the process of changing your home campus, contact

Students from Fond du Lac or Fox Cities, who want Oshkosh as their new campus, must have completed 12 credits and have a 2.0 cumulative GPA. Change of campus approval is conditional based upon a student’s academic standing.

Please note, it is important that before completing this form you speak with your advisor to ensure class offerings are still available in regards to meeting your plan of action.

Courses at each campus may be assessed different tuition. Current rates are found at:

  • Campus You Intend to Change To
  • For students at UW-Fox Cities or UW-Fond du Lac, please indicate if you have or are in the process of completing your Associate of Arts and Sciences degree.
  • For students at UW-Fox Cities or UW-Fond du Lac, please indicate if you plan to complete your Associate of Arts and Sciences degree.
  • i.e. Fall, Spring, Summer and year (Fall 2019)
  • If you are unsure, list undecided
