Report a Safety Concern

Use this form to report any of the following:

  • Safety Concern: A potential source of injury or property damage; an “accident waiting to happen” situation.
  • Close Call: An incident that nearly resulted in injury or property damage, but didn’t. You were lucky! However… you see that the incident could happen again, and the next person may not be so lucky.
If you believe there is immediate danger of severe injury or damage, call 911 or UWOPD at ext. 1212 before filling out this form.

 

Please note: Severe incidents are to be reported directly to your supervisor, either immediately or after any urgent medical treatment is applied.
