Welcome to the Titan athletics family!

The sports medicine staff at the University of Wisconsin Oshkosh look forward to serving you by caring for any medical needs that you may have in the future. Contact us if you have any medical issues. We are here to help and provide quality health care to all Titan athletes.

All new UWO athletes (freshman/ transfers) must complete all necessary medical information prior to any team activity/ practice. The video to the right details all necessary information needed to be completed by new athletes. Please contact your teams Licensed Athletic Trainer with any questions. Below is a list of necessary medical items needed:

1. Physical Exam Form/ Sickle Cell Trait Verification Form. This is to be completed by a physician and either physically handed in, faxed to 920-424-1068 or may be uploaded into your ATS account under the “efiles” tab. The first page of the form is the actual physical exam record. In order to comply with NCAA recommendations, you are required to provide notification from a physician as to the results of the sickle cell trait test on page two prior to participation in UWO intercollegiate athletics. Physicians may look at your birth records to get your sickle cell trait status. This form can be found below. 

2. ImPact Concussion Test- All new athletes will be required to take the computerized ImPact concussion test. Depending on your sport, you may be sent a link via email to complete this test at home prior to reporting or you may do as a team on the first day of practice. Your teams Licensed Athletic Trainer will give direction on this test. 

3. SWAY Balance Test- All new athletes will complete this test when arriving to campus and prior to any team activity. Your teams Licensed Athletic Trainer will give directions on when this test will take place. 

4. ATS Medical Records– ATS stands for Athletic Trainer Software and is our electronic medical records system. All new athletes must complete a variety of forms and submit athlete information. Your teams Licensed Athletic Trainer will create an ATS account for you to access the system. If they have not created an account, please contact your teams Licensed Athletic Trainer. Access to the login page is below. 

 

ATS Medical Website Access

Note: An account must be set up prior to accessing the medical website. Please contact your sports athletic trainer to complete this process. If this process is not performed, you will not be able to access ATS. The complete list of sports athletic trainers can be found by clicking here.

The physical exam form along with sickle cell trait verification form may be uploaded into ATS under the “e-files” tab or they may be faxed to us at (920) 424-1068 or mailed to the following address:
