The University of Wisconsin-Fond du Lac is seeking nominations for its 12th annual Distinguished Alumni Award.

Nominations must be submitted by Tuesday, May 5. A nomination form is available online.

The award has been established to honor and recognize UW-Fond du Lac alumni who have attained notable achievement in their career field, demonstrated philanthropic or public service activity in support of the community or provided inspiration to others.

Nominees must have attended UW-Fond du Lac at any time since the campus opened in 1968 for at least two semesters.

Created in partnership with the UW-Fond du Lac Foundation, the Distinguished Alumni Award will be presented at the annual donor and scholarship recognition reception on Sept. 30. The recipient of the award will also be honored with a personalized brick in the Foundation Celebration Walk and will have their name engraved on a perpetual plaque displayed in the University Center Commons.

Previous Distinguished Alumni awards have been given to Brenna Garrison-Bruden (2004); Don Noe (2005); Paul Rosenfeldt (2006); Robert Carew (2007); Oscar Kraus (2008); Sandi Roehrig (2009); Mark Strand (2010); Kim Mooney (2011); William (Bill) Lamb (2012); Scott Wittchow (2013); and Paul Thelen (2014).
