Eligible members of the community may request a COVID-19 vaccination appointment for next week on a digital form now open on the University of Wisconsin Oshkosh Titans Return website.

Only members of the public who meet eligibility requirements may request an appointment. Vaccinations will take place at the Culver Family Welcome Center, 625 Pearl Ave., Oshkosh. The vaccination center is a partnership between UW Oshkosh, Advocate Aurora Health of Oshkosh and Winnebago County Public Health Department.
Wisconsin is currently vaccinating frontline healthcare workers, residents and staff in skilled nursing and long-term care facilities, police and fire personnel, correctional staff and those age 65 and over.
When the request limit has been reached, users of the form will receive a message that the site has reached capacity and is unable to accept new requests. Capacity is expected to be reached quickly. Those who successfully request an appointment will be contacted by Advocate Aurora via email early next week regarding next steps. Questions about that process should be directed to Aurora at (920) 456-6000.

Hello, Susan: To receive a vaccination at the community site at UWO’s Culver Family Welcome Center you will need to make a reservation online at https://uwosh.edu/titans-return/vaccination/. Once appointments are full for the week, the form will be closed. Check back on Friday afternoons as appointments open back up for next week.
I signed up using the form on the website on Friday Feb 19 and have not been contacted by aurora as stated in the instructions. What happened?
When will you be getting the Johnson & Johnson vaccine in ? I would like to apply for that shot when it is available. Please advise.
We are not notified about type of vaccine that we receive until allotment numbers are received, typically on Fridays. If we are aware that we have the Johnson & Johnson vaccine, we will note that information on the website that lists information related to the Culver vaccination center.
When you request an appointment, that request is forwarded to Advocate Aurora Health. If you have not received an appointment it may be because of shipment delays that impacted testing last week. All those who signed up for testing on Feb. 19 should receive an appointment for this week. If you do not hear anything, please contact Aurora.
