Field Placement Site Information Form

To update your information or be considered as a potential placement for our student interns in the future, please submit this form. If you offer multiple programs, please complete one form for each.

• Placements of students are never guaranteed, nor are you obligated to accept a placement.
• Whenever your agency is selected by a student, our office will send a placement request to your site. You may either accept, decline or accept pending interview any placement request.
• Serving as a potential placement assures that your agency will remain on the list of options.
